RESUMO

The study has established that during a progressive tuberculous process, immunological changes appear as significant increased proinflammatory cytokine production that leads to progressive destructive processes in the lung parenchymal lesion areas. Inclusion of lymphotrophic chemoimmunotherapy with roncoleukin into the complex therapy of patients with progressive pulmonary tuberculosis permits the optimization of the results of treatment in patients with progressive pulmonary tuberculosis due to the target delivery of antituberculous and immunoactive agents.

RESUMO

The data characterizing the specific features of immune response in relapsing and atypical course of herpetic infection, including changes in the characteristics of cell-mediated and humoral immunity, interferon and cytokine status (alpha- and gamma-interferon, tumor necrosis factor alpha, interleukin-4), are presented. The variants of the pathogenesis of immunological disturbances in relapsing and atypical herpesvirus disturbances are proposed.

RESUMO

AIM: To validate optimal cost-effective pharmacological policy in combined therapy of elderly hypertensive patients. MATERIAL AND METHODS: In 150 patients of a day hospital aged 55-74 years with at least stage II arterial hypertension of 4-40 years of duration a study was made of the cost efficacy of their combined treatment. The patients received lisinopril, metoprolol, nifedipin, enalapril, hypothiaside, indapamid. Clinical efficacy was assessed by the following criteria: a fall in systolic arterial pressure to at least 139 mm Hg or at least by 20% of the baseline, a fall in diastolic pressure to at least 89 mm Hg or 10% of the baseline. Economic parameters included cost of the disease, cost/effect values. RESULTS: Combination egilok+hypothiaside was the cheapest but the least effective. Diroton+indap combination was cheaper than less effective combination cordipin-retard+hypothiaside and renitek-hypothiaside. Pharmacological and cost efficacy of diroton+indap combination was the highest. CONCLUSION: Combination of lisinopril with indapamid was most adequate as to its cost and effect and can be used as a medication of the first line in elderly hypertensive patients.

The experiments on rats with model traumatic disease showed that melatonin administration produces a dose-dependent positive effect, decreasing the degree and duration of disturbances in platelet hemostasis.

RESUMO

Pineal gland phylogenic antiquity, its involvement in the stress reaction and many-sided activity of its compounds permit to consider the gland to be the organ taking an active part in the body protection against injury, which is essential for everyone since its appearance on the planet. The available data prove that almost all the reactions developing in the organism from the moment of trauma up to the complete wound reparation are under the pineal control. Being always an urgent one the problem of traumatism increased nowadays because of pineal dysfunction. It makes the premises to apply pineal secret products in traumatic disease as a kind of replace therapy that is suggested to be not less successful than glucocorticoid administration in shock.

RESUMO

Shockogenic sublethal mechanical multiple injuries were induced in outbred albino rats according to the method developed by Noble-Collip and periodontal tissues were examined histomorphologically in various periods after shock. Atrophic changes in periodontal tissues and dental pulp progressed starting from day 1 and reached the maximum on day 28. The greatest shifts were observed in the gingivodental epithelium (hyperkeratosis), periodontium (lymphocyte and macrophage infiltration), alveolar bone (lacunar resorption), and pulp (odontoblast reduction and decreased pulpocyte count). Phasic reactive changes in the periodontium after shock and trauma require timely correction.

Up to 1920 doctors in Siberia were trained only at the Medical Department of the Tomsk State University, from which about 40 specialists graduated annually. Due to the most severe sanitary and epidemiological situation and a critical lack in doctors in 1920, it was decided to open a medical institute in Omsk.

RESUMO

The changes occurring in the proliferation and differentiation of pluripotent hematopoietic stem cells under the action of different doses of benzylpenicillin and aspisole were investigated by using models of sublethally and lethally irradiated mice. The mitogenic and mitostatic dose-dependent effects of the drugs were revealed at the very early stages of immunogenesis and hemopoiesis. These effects show themselves in different ways (depending on the predominance of T- and B-lymphocytic conditions for T-B-cooperation and so on.

RESUMO

A total of 233 patients with acute peritonitis who in the postoperative period were subjected only to conventional complex intensive care (Group I) have been examined. Changes in medium molecular weight polypeptide level, relative and absolute monocyte and granulocyte content, phagocyte number and index, blood phagocytosis capacity and metabolic potential of macrophages in spontaneous and activated NBT test have been established. Integral tetrazolium neutrophil activity was different in subgroups of patients with (Group I) and without (Group II) complications. The factors which play the key role in the formation of pyoseptic complications have been identified as were diagnostic-prognostic tests evaluating their critical level. Patients from Group II with a high level of medium molecular weight toxin, besides conventional therapy, were subjected to hemosorption which brought about a positive clinical and laboratory effect.

RESUMO

As exemplified by iron deficiency anemia, the study was made of effect of iron deficiency on interdependence of immunity parameters. It is noted that patients with definite forms of the anemia showed less numerous correlations than in controls. The emergence of complications increased the number of persistent correlation pleiad compared to controls. Moreover, correlation and variance analyses suggested a correlation between immunity and iron deficiency parameters.

RESUMO

Examinations of 81 patients and 30 healthy donors have revealed a variety of immunologic disturbances in different forms of chronic nontumorous conditions of the parotid glands. Lymphocyte sensitization to salivary gland antigens was observed in all the patients, but was the most marked in those with Sjogren's disease. The studies permit considering the detected shifts in the immunity system an important component of the pathogenesis of chronic nontumorous diseases of the parotid glands. These findings may be useful in the differential diagnosis, prognosis, and adequate therapy of salivary gland diseases.

Morphological and biochemical indexes of peripheral erythron compartment have been analysed 1, 3, 7 and 14 days after severe mechanical traumas of various locations in human patients, after 5- and 10-min clinical death in dogs and after rotatory polytrauma in rats. It has been shown that rehabilitation period along with usual anemization is characterized by enhanced penetration of newly formed erythrocytes (hyperchromic, fetal, lipoperoxidation-resistant forms) into the circulation. In pathologies with predominant damage of the central nervous system regenerative response of the bone marrow is less marked and normalization of hematologic parameters is less prompt. As erythron changes always take place in terminal and postresuscitation conditions, irrespective of the etiology and species, they are regarded as a typical pathological process.

The dynamic study of a number of clinical and laboratory findings in 107 patients with acute peritonitis have shown that with the current complex infusion therapy the onset of postoperative complications is to a great extent mediated by high toxin level--medium molecular mass polypeptide, inhibiting the processes of protein synthesis and consequently causing changes in the relationship between adaptation compensatory reactions, which demonstrates the necessity of additional active detoxicating therapy, i.e. hemosorption (HS). HS was performed to 126 patients and its efficacy in pronounced toxemia was established. The data obtained make it possible to consider HS as an effective means preventing the onset of postoperative complications in acute peritonitis.

RESUMO

An examination of 65 patients with acute diffuse peritonitis has shown that pronounced T-immune deficiency is observed in the postoperative period of acute peritonitis. Hemosorption was shown to be efficient in patients with high content of toxin which allows considering the method of extracorporeal detoxication as effective means of immune-correcting therapy in peritonitis patients with high level of toxinemia who make up a group of risk.
